You "beat" the level after killing the boss baddie; the guy with his picture in the level tile. Sometimes though it will award you a 'Cleared' even though you haven't yet killed the boss.
As far as the stability is concerned, I would just say it was bad luck as you said. I've had the game for almost a year and back and forth from external to internal with no issues yet.
