Unplugged from power or pushed the power off button on the ouya.
I had the same issue and it was gone after I restarted the ouya.

I own an ouya with 2 controllers. They both get out of sync after about 30 to 40 minutes of playing a game.
- doenst matter which controller I use (singleplayer games)
- affect both controllers (multiplayer games)
Out of sync
- controller gets a lag, lag gets bigger and bigger
- controller simulates input (moves up, down, left right by ghost), stops after 3 to 5 seconds with that ghost moves
This can be reproduced with EVERY game, even with XBMC.
What to do?

The standard things to try on an Ouya is to turn it off, and back on again, via the top button (hold for a couple seconds then it will prompt you to turn it off), to clear the cache of the Ouya Launcher (Manage -> System -> Advanced -> Apps -> All -> scroll down to Ouya Launcher and then hit Clear Cache) and to make sure Destroy Programs On Exit is selected in the Developer Settings.
I have the same problem, maybe not as predictable as the OP described, but I get if often. More often when playing local multiplayer.
I shut down the power supply of my OUYA over night, so I cold boot every day.
A few observations regarding this behavior:
- My console is 2.5m (about 7 feet, I believe) away from me. No other bluetooth devices.
- The old "get closer to the console"-trick seems indeed to work (I'm not sure because the problem goes away by itself anyway, after 2 - 5 seconds of random character movement). If I have the controller on my lap with my knees up, and the lag happens, it seems to get better once I jolt forward and strech my arms in the direction of the console.
If anyone considers this a "solution", please think again.
- Knowing that some people don"t experience this issue at all, I deduce there's a variable involved other than distance. Hardware of controller? Buggy bluetooth module? Magnetism (speakers)? Controller firmware?
Ouya support said to elevate my console and play near to it. Again, I don't consider that a solution and it can't explain why some people with I'm sure identical setups play without problems.
I think this urgently needs to be addressed, and I'd love to get an explanation and possible fix. I'm happy to wait if it's a software thing, but otherwise, I'd like to know what to replace.
Have you tried laying the Ouya on its side with the top facing you? That works for some people.
I tried that, with little notable difference. As the problem is most prominent when playing multiplayer, I think I'll take the OUYA out of it's case before the next MP session. If it's gone then, at least I know it's really the connection.
I think I'm one of the lucky ones in that I rarely have any controller problems. Update us and let us know how taking it out during multiplayer worked for oyu.
On all the controller lag issues I have read (and I have had my share of controller lag as well, no doubt), has any confirmed if you get this lag with third party controllers? I don't see people post much about that scenario. I know or me, the last time I played Bomb Squad with 2 other controllers (PS3), it seemed that I would get the occasional lag (Ouya), but the other two never indicated that they did. I'm trying to see if this is more about the actual Bluetooth on the system, or the actual controller itself for Ouya.
