The actual transaction is done through the ouya framework. Once you click on something that initiates a transaction, a pop up from the framework comes up that is outside of the control of the application so that you can approve the transaction. You can also set a pin so that you cant just fly through that approval by accident. I suspect that you are asking if a game can pull a fast one on you and rack up charges without you noticing that its happening and the answer is no.

I was going to download Sonic,but it states in-game purchases . From what you have said,I would have to physically agree to any future purchases ?. I just wouldn't want to find out later that I have spent X amount on extras .

No, as dra6onfire said that won't happen, you can rest assured.Be careful though that Sonic 4 episode 1 and Sonic CD will revert to demo mode as long as your connection goes down or the Ouya servers are unavailable. Sonic 4 ep 2 is not affected by this bug.

When they say "In App purchases" in the game details page, it USUALLY means that it is just pay to unlock. Ie: one time price after the demo. If you were ever to remove or reinstall the game, it still recognizes that you purchased said game.
There are some, like Skiing Fred, where there are simply In app purchases to get new items and such.

Games that are free with donations also show in app purchasing too. I have always had a pin on any purchases as its just another fail safe so you don't spend any cash you didn't want to.

To clarify: It's confusing from the descriptions what kind of in app "purchasing" a game has before you try it. It's not confusing when and whether you're actually being asked to make a purchase.
Download and play with confidence. You'll know when a game wants your money. But do set a PIN if you're worried you might reflexively confirm the purchase without meaning to (it's never happened to me personally).
